SOCIAL SERVICES
LifeCare Social Service Department provides services in acute
care, long term care, home health care and hospice for all ages
from infancy through geriatric. The Social Service Department
also is responsible for the discharge planning process and
coordinates the Swing-bed program.
Scope of Care Provided:
Assess patients/residents/clients of all ages to determine
discharge needs and assist in the
coordination of services available.
Follow mandated reporting guidelines in the event of suspected
physical/emotional abuse or
neglect, sexual abuse or financial exploitation.
Make proper referrals or provide appropriate counsel for
patients/residents/clients according
to their assessed needs.
Coordinate and facilitate patient/resident/client family
conferences to assist them in decision
making or problem solving.
Provide education to staff regarding patient/resident/client
rights, including the Minnesota
Vulnerable Adult Act and the Federal Patient Self Determination
Act and the Minnesota State
Reporting of Maltreatment of Minors Act.
Maintain a Quality Assessment process specific to Social
Services with a CQI philosophy
consistent with Roseau Area Hospital & Homes, Inc.
Skill Level of Personnel
All SW personnel are employees of RAH&HI and must have an
undergraduate degree in SW from an accredited SW college and in
addition must be licensed through the Minnesota Board of Social
Work. It is the goal of the department to be able to provide
consistent and timely services to all areas of the facility.
Therefore, in order to do this, the department has developed a
plan to cross-train it's employees to more than one area, i.e.,
LTC and HHC, HHC and ER, acute care and LTC. Each employee must
meet the continuing education requirements of licensure and in
addition meet all on-going competencies appropriate to the
assigned work areas.
Staffing Plan
Acute Care (including ER) - 40 hours per week; Sheltering Oaks
Manor Nursing Home – 40 hours per week; Greenbush Nursing Home -
40 hours per week; Roseau County Home Health Care – as needed;
Northern Communities Hospice – 16 hours per week.
